For a copy of your medical records:
- Please click the link below to submit a medical records request; Do NOT submit your request in person.
- You will be asked for payment on the 2nd page, and the request will be submitted once paid.
- You should receive your records within 5-7 business days of paying for your request.
- Your medical records will be delivered via an email that contains a link where the records can be viewed and downloaded digitally to your computer, tablet, or phone.
